If your bill amount exceeds the costs, you will be responsible for the balance amount. We accept cash, MasterCard and Visa as well as a personal cheque. Payment is required before discharge. |

- TPA ID CARD OF POLICYHOLDER
- CURRENT YEAR INSURANCE POLICY COPY
- PHOTO IDENTIFICATION OF PATIENT
a) ADULTS
Driving license, Voter ID card, Passport, Election card, Government employee card, Photo affixed on policy copy and duly signed by insurance company OR Previous TPA ID card Of the patient.
b) CHILDREN
Photo ID of school, photo affixed on policy copy duly signed by
the insurance company
